Dear colleagues, please read the following information before filling the form.

The manufacturer of the implant system IMPRO provides a lifetime warranty on implants. This is due to the high accuracy of production, the use of the most modern equipment, and the use of high quality raw materials. The implants have passed all the necessary researches, including research on strength and reliability. For example, implants with a diameter of 3.6 mm are able to withstand a load of up to 1000 Ncm, which is confirmed by the results of the crash test (detailed information can be found here). The manufacturer guarantees the accuracy and reliability of all system components, as they comply with European quality standards.

In case of failure of the implant, the manufacturer guarantees its replacement if the test results show that the failure occurred due to the manufacturer’s fault (violation of the raw material processing technology, excessive thinning of the walls during processing, etc.) or the reason of the failure incomprehensible. In case of implant failure, the manufacturer guarantees its replacement if the test results show that implant failure could have occurred due to the manufacturer’s fault (defects on the implant surface finish, etc.). Implant osseointegration depends on a large number of factors; therefore, failure cannot be considered unambiguously as the manufacturer's fault.
The experience of using the IMPRO implantation system shows an extremely low implant failure rate, both in the early and in the long term after implantation (on average, from 0.1 to 0.5%). In any case, the manufacturer is not responsible for the possible consequences of a breakdown or failure of the implant.

In case of failure of the implant, please fill out the Failed implant return form of the implant and send it to the address of the representative of the IMPRO implant system in your region or to the address of the exclusive representative of the IMPRO implant system in Germany (info@impro-impant.com). You must also send the following information:
Initial X-ray (Pano, CLCT or periapical) - made prior to implantation required*
X-ray image (Pano, CLCT or periapical) immediately after implantation is required*
X-ray (Pano, CLCT or periapical) before installing the healing abutment - mandatory if the implant is failed during the installation of the healing abutment or after fixing the crown
X-ray image (Pano or periapical) after fixing the prosthetic structure (abutment) - mandatory if the implant is failed after fixing the prosthetic structure (with abutment)
We also recommend that you take advanced training courses at an authorized training centers, which will help to avoid mistakes and will achieve high results of implant survival.

Limit on the replacement of implants per doctor per year: 1. Lack of primary stability no more than two cases per year.
2. Stuck in a packaging container no more than two cases per year.
3. Inaccurate actions of a doctor (loss of sterility) of not more than one case per year.
In the case of exceeding cases for these positions for the year, the implants will not be replaced.

The application will be considered only with the full filling of all points and providing all the required images (x-ray).
